Double living liver transplant, the record-breaking operation in Padua

Another avant-garde intervention in transplant surgery at the Padua hospital. After the first heart transplant with a 20-minute stopped heart, from a deceased donor, performed in early May, this time it is a double liver transplant from two different living donors.

A 52-year-old woman with metastatic adenocarcinoma of the colon and on whom chemotherapy was no longer effective received two portions of liver from living donors. The woman was saved by her two nephews, aged 28 and 30, who underwent explantation.

Unlike traditional living donations, when 65% of the liver is exported to a single donor, the two young people decided to donate 25% of their liver, allowing for a double transplant to reconstruct the aunt’s organ.

For the woman, in fact, said the professor Umberto Cillo “Three lines of chemotherapy weren’t enough. That’s why we considered transplantation.”

Living liver transplantation: when it can be done

“This significant intervention – said the director of the National Transplant Center Massimo Cardillo – is part of a program, living liver transplantation, which has already started in Italy and on which significant experience has already been gained. A type of operation that can be done if there is a donor available within the family. In particular, the innovation here arises from the fact that the donation procedure used two different donors for a single patient. This – concluded Cardillo – to minimize the invasiveness of the intervention in the donors and at the same time collect a sufficient amount of tissue for the functional recovery of the liver in the transplanted patient”.

The part removed from the donors will grow back in a month

The patient’s two grandchildren donated the left side of their liver: the two lobes, which will regenerate in 4 weeks. The pieces of tissue were removed, surgically reversed and transplanted onto the woman. the operation lasted 20 hours and was attended by over 50 professionals from the University of Padua hospital, led by Umberto Cillo.

Over 6 thousand transplants have been carried out in the last 10 years: in 2022 alone there were 604, of which 369 interventions referable to Padua (equal to 61%). In the first 6 months of 2023, the region contributed to 16% of the total transplants carried out on national soil (315 transplants).
